Does anyone know the part number for the heater hoses that run from the block to the firewall?

Some heater hoses are listed as 5/8" and another one is listed as 3/4". You will have to measure what size that you need. I will take a guess and say that yours is might be 5/8" hoses.

 
5/8" Hose:
CONTINENTAL 65004 Silicone hose must be used with special clamp; I.D. 5/8" - Priced Per Foot 5/8" x 7'

GATES 28417 (Alternate/OEM Part Number(s): 28411, 28415, 28491, H159)
Straight Heater Hose (Standard); 5/8" (60 psi) - Priced Per Foot, Cut to Length, 4-1/2"  
 
ACDELCO 30124 (Alternate/OEM Part Number(s): #30127, 88909065) Professional; Straight; 5/8" (60 psi) - Priced Per Foot, Cut to Length, 4-1/2" x 50"

3/4" Hose:
GATES 28412 (Alternate/OEM Part Number(s): #28418, 28492, H152) Straight Heater Hose (Standard); 3/4" (50 psi) - Priced Per Foot, 3/4" Polybag Htr Hose
